Immediate Prohibition Notice PN302917571 - prohibiting the operation of the Superbob ride in reverse mode.


Thunder Mountain
Notice 302738494 served against Andrew David Harniess on 21/07/2010.
20/7/10 - Immediate Prohibition Notice on 4th car of Thunder Mountain ride, broken passenger restraint. Weld on the right hand side of the passenger restraint had failed so the restraint was not able to secure passengers on the supercar mini roller coaster, called Thunder Mountain, Serial Number Y1293 on vehicle C738 YNW, leading to a risk of passenger ejection.

Ground Shuttle Ride
Notice 300298199 served against Riggsed Limited on 08/08/2006.
Prohibition Notice to stop use of ground Shuttle ride (telehandler with passenger carrier fitted to the forks) at Diggerland site, Strood. Route followed by ride takes ride close (approx 1 to 1.5m) from hollow in ground; driver has direct field of view obscured, effectively relying upon memory of route and side view from cab; seat belts can be unlocked by passengers at any time despite RA requiring them to be key-released by marsall. Carrier not fitted with any form of RoP structure.

Pinfari Zyklon 40 Rollercoaster, Maxwell Waltzer
Notice 300183631 served against Killarney Springs Family Leisure Park Limited on 18/05/2006
Prohibition Notice DJH / 090506/01 issued on Pinfari Zyklon 40 Rollercoaster. Killarney had failed to maintain and update the restraint and weights on the cars so far as is reasonably practicable to ensure members of the public are not exposed to risks to their health and safety. Improvement Notice DJH 180506/01 issued on Pinifari Zyklon Rollercoaster, failed to adequately maintain and update so far as is reasonably practicable to ensure employees and members of the public are not exposed to risks to their H&S. 1) the worn and damaged wooden walkway access to the track, 2) the missing edge protection from the working platform above the station, 3) the damaged exit steps from the mid rail. Comply 18th June 2006 Improvement Notice DJH180506/02 on the Maxwell Waltzer, failed to adequately maintain so far as is reasonably practicable to ensure employees and members of the public are not exposed to risks to their H&S.1) the central decking (dancer boards ) need repair, 2) one platform (No13) requires replacement, 3) properly fence off the rear 2 access and steps. Comply 18th June 2006 Improvement Notice 180506/03 failed to have effective arrangements in place to ensure employees are of appropriate age to operate the rides, have received adequate appropraite health and safety training, have received appropriate ride operation training in accordance with HSG175, and have an appropriate maintenance recording procedures. Comply 18 June 2006.

Waltzer

PN 300208141 Unsafe deck grating on waltzer, served as P/JMCE1 following accident.

Air Diver (Super Booster)
Immediate Prohibition Notice 300143098 served against Brighton Marine Palace & Pier Company Ltd on 19/04/2006. S&S Design review not in place - as member of the public hurt whilst on device. Failure to ensure a suitable and sufficient design review in place for the device, therefore failing to ensure the safety of people riding it, the Air Diver (Super Booster) due to mechanical, electrical or structural failure. Telephoned Richard James associates to confirm if design review had been completed, stated that it had not.

Dragon Coaster
Improvement Notice 20005748 served against LEGOLAND Windsor Park Ltd on 08/11/2005
No design review on the Dragon Coaster ride. The Dragon is the biggest and fastest roller coaster at Legoland Windsor. The ride is designed to look like a dragon and it shoots in and out of the "castle". It has green carriages and each car carries two people.






Twister Ride
Notice 10008022 served against The Showmen's Guild of Great Britain on 27/09/2005.
A twist ride positioned at the eastern end of Barton Street in Tewkesbury reduced the access to approximately one metre thereby preventing access for emergency vehicles and evacuation of a crowd. There had been negotiations over moving this ride to a different position for 12 months.

Dodgem Cars

Electric supply to Dodgem ride not maintained or constructed to prevent danger, also unsafe connections, inadequate fusing and earthing.
